When you refer a client to a personal injury chiropractor, the care matters and so does the record. Physical Medicine & Rehabilitation of NY & CT (PMR) provides chiropractic care, physical therapy, physiatry and diagnostic testing at two offices in Danbury, CT and Brewster, NY. This page is for attorneys, paralegals and case managers.

Personal injury chiropractor and PT documentation
Each client’s chart includes:
- Initial evaluation: accident and symptom history, exam findings such as range of motion, strength and neurological checks, any X-ray or EMG results, a diagnosis and a treatment plan with goals
- Visit notes: the care provided at each visit and how your client responded
With chiropractic care and personal injury physical therapy in one practice, records for both can come from one source.
Requesting medical records for injury claims
Include a HIPAA-compliant authorization signed by your client, the dates of service you need and the records you want, such as evaluation and visit notes, imaging reports or itemized bills.
Referring a client
PMR accepts attorney referrals for car accident and other personal injury cases.
Ask your client to bring photo ID, the claim number, the insurer’s name and adjuster contact, and your firm’s contact details. Once your client signs an authorization, our staff can discuss scheduling and records with your office.
New York no-fault and Connecticut at-fault cases
In New York, no-fault (PIP) provides up to $50,000 per person for necessary care and lost earnings, whoever caused the crash (11 NYCRR 65-1.1). With an assignment of benefits (NF-AOB), PMR bills the insurer directly at fee-schedule rates (11 NYCRR 65-3.11). Physical therapy is covered only on referral (NY Insurance Law 5102). Connecticut is an at-fault state: injured people claim against the driver who caused the crash (OLR report), and PIP hasn’t been required since January 1, 1994 (CID Bulletin PC-22). Care is usually paid by optional MedPay coverage, health insurance, a claim against the at-fault driver, uninsured or underinsured motorist coverage, or an attorney’s letter of protection. If a client needs emergency care
PMR treats musculoskeletal and movement problems, so a personal injury chiropractor isn’t the right first stop for emergency symptoms. Send a client to the ER or call 911 for:
- A severe or worsening headache, confusion or repeated vomiting after a crash
- New numbness or weakness, or loss of bladder or bowel control
- Chest pain or trouble breathing

Do you accept letters of protection?

Will your records support my client’s claim?
Our records describe your client’s injury, the care provided and how your client responds, as accurately as we can. We don’t tailor documentation to a legal strategy, and we can’t predict how an insurer, arbitrator or court will view a case. What we can offer is a clear, consistent record of care from evaluation through discharge.
What if my client needs care PMR doesn’t provide?
Our scope is chiropractic care, physical therapy, physiatry and related musculoskeletal services. If your client’s symptoms point to a problem outside that scope, we coordinate with your client’s physician or refer your client to an outside specialist, so the right provider is involved. Your office can request those outside records directly from that provider.
