Whiplash can turn a minor fender bender into weeks of neck pain, stiffness and headaches. At Physical Medicine & Rehabilitation of NY & CT (PMR), whiplash treatment starts with a careful exam and focuses on helping you move comfortably again, using chiropractic care, physical therapy exercise and massage. What is whiplash?

Whiplash is a neck injury caused by a fast back-and-forth movement of the head, like the crack of a whip. It most often happens in rear-end car crashes, but sports collisions, falls and other sudden jolts can cause it too. The motion can strain neck muscles and tendons, sprain ligaments and the small joints of the spine, and irritate nearby nerves.

Whiplash symptoms

Common whiplash symptoms include:

  • Neck pain and stiffness, often worse when you turn or tilt your head
  • Headaches, frequently starting at the base of the skull
  • Pain or tightness in the shoulders, upper back or arms
  • Dizziness
  • Tiredness, poor sleep or trouble concentrating

Why symptoms can be delayed

Many people feel fine at the scene and only notice pain hours or a couple of days later. The body’s stress response can mask discomfort at first, and injured tissues often stiffen overnight. Late-starting neck pain after a car accident is still worth having checked.

When to get urgent care

Go to the emergency room or call 911 if neck pain is severe after a hard impact or comes with any of these:

  • A sudden, severe headache, or a headache with vomiting or confusion
  • Numbness or weakness in an arm or leg, or trouble walking or keeping your balance
  • Trouble speaking, swallowing or seeing
  • Loss of bladder or bowel control

How whiplash treatment works at PMR

Whiplash treatment at PMR is built around your exam findings and daily demands, such as driving, desk work or lifting your kids. Plans aim to calm pain, restore motion and rebuild strength, and they change as you improve.

A careful evaluation

Your provider asks about the crash and your symptoms, then checks how your neck moves, where it’s tender, and the strength, reflexes and sensation in your arms. This screens for signs of a fracture, ligament or nerve injury, or concussion. If something looks concerning, we order X-rays or other imaging, or send you to a physician first.

Staying active

For most whiplash injuries, gentle movement tends to help more than rest. Your provider may suggest easy range-of-motion exercises, short walks, and heat or ice for comfort.

Chiropractic care

A whiplash chiropractor uses hands-on techniques such as joint mobilization, gentle adjustments when appropriate and soft-tissue work to ease stiffness and help your neck move more normally. Early care is usually gentle and matched to how sensitive your neck is.

Physical therapy exercise

Physical therapy builds the strength and control your neck needs for daily life. A typical program includes deep neck flexor exercises, shoulder blade strengthening, posture training and a gradual return to reaching, lifting and turning. PMR’s on-site gym gives you space to practice with guidance.

Massage therapy

Massage therapy can relax tight, guarded muscles in the neck and shoulders and may make exercise easier. Some patients also add acupuncture or trigger point therapy to their plan.

How long does whiplash take to heal?

Recovery varies. Many people improve within a few weeks, others need a few months, and some people have symptoms that last longer. Your progress is rechecked at regular visits, and if it stalls, your team may adjust the plan or bring in our pain management and physiatry providers.

Frequently asked questions

Should I see a chiropractor for whiplash?

A chiropractor can be a good first stop for whiplash once emergency warning signs are ruled out. At PMR, chiropractors screen your neck carefully, use gentle hands-on care when it’s appropriate and work with physical therapists on your exercise plan. If your exam raises concern for a fracture, nerve damage or a concussion, they’ll send you to a physician.

Can whiplash cause headaches and dizziness?

Yes. Headaches are among the most common whiplash symptoms, often starting at the base of the skull and spreading toward the forehead. Some people also feel dizzy or unsteady, especially when turning their head. Because these symptoms can overlap with a concussion, get them checked. PMR also offers vestibular rehabilitation, which may help some types of dizziness.

Should I wear a neck collar?

For most whiplash injuries, a soft collar isn’t needed, and wearing one for long periods can leave your neck weaker and stiffer. Gentle movement within your comfort range usually helps more. The exception is a suspected fracture or serious ligament injury, when an emergency physician may place you in a collar until imaging is done; follow that advice until you’re cleared.

How soon should I start whiplash treatment?

Ideally within a few days of the injury, even if symptoms are mild. An early evaluation can rule out more serious problems, give you a safe plan for staying active and record your symptoms from the start. If pain begins days later, it’s still worth getting checked, since delayed whiplash symptoms are common.
