Car accident, workers’ comp and personal injury claims
New York car accidents (no-fault)
New York is a no-fault state. No-fault coverage (PIP) pays up to $50,000 per person for necessary care and lost earnings, whoever caused the crash (11 NYCRR 65-1.1). File the NF-2 application with the insurer within 30 days of the accident (DFS no-fault forms). With an assignment of benefits (NF-AOB), PMR bills the insurer directly at fee-schedule rates (11 NYCRR 65-3.11). Physical therapy is covered only with a referral (NY Insurance Law 5102). See New York no-fault car accident care.
Connecticut car accidents (at-fault)
Connecticut is an at-fault state: injured people claim against the driver who caused the crash (Connecticut OLR report), and PIP hasn’t been required since January 1, 1994 (CID Bulletin PC-22). Care is usually paid by:
- MedPay (optional coverage)
- Health insurance
- A claim against the at-fault driver
- Uninsured/underinsured motorist coverage
- An attorney’s letter of protection
See Connecticut car accident care.
Workers’ compensation
- New York: Care must come from a Board-authorized provider, and you pay no copays (WCB health care). Tell your employer in writing within 30 days and file Form C-3 within 2 years (injured workers toolkit). See New York workers’ compensation.
- Connecticut: Report the injury to your employer right away and file Form 30C within 1 year (3 years for an occupational disease) (CGS chapter 568). If your employer has an approved medical care plan, you must use its providers, so ask us whether PMR is part of it. See Connecticut workers’ compensation.
Personal injury
PMR also accepts personal injury cases. If an attorney represents you, bring their contact details and any claim number to your first visit.
Frequently asked questions
Will I owe a copay or deductible?
It depends on your plan, since each health plan sets its own copays, coinsurance and deductibles. Our insurance and billing team explains your benefits and eligibility at your first appointment. Workers’ comp patients in New York pay no copays. For questions about a bill, call our billing office at (203) 744-8014.
What if my plan isn’t listed?
Plan lists change over time, so it’s worth checking. Send the coverage-check form on this page with your insurance carrier’s name, or call our billing office at (203) 744-8014. If we don’t accept your plan, ask about self-pay options before you book.
Should I tell you if my visit is for a car accident or work injury?
Yes. Tell us when you book, because car accident and workers’ comp claims are billed differently from health insurance, and New York and Connecticut rules differ. Have your claim number, the insurer’s name and phone number, and your employer’s or attorney’s contact details ready.
